How to Become a Patient

In order to become a Wake Smiles patient, you must be an uninsured Wake County resident who meets income requirements and be referred by one of our participating agencies.

To be a patient at Wake Smiles, you must be a Wake County resident, uninsured and low income. If you have Medicaid, Medicare or any health or dental insurance you will not qualify for our services. There is a $40 administrative fee for each appointment. We accept credit/debit cards.

Patients MUST be referred by the following participating primary care clinics:

Alliance Medical Ministry

  • Age 18 or above
  • No health insurance (no Medicaid/Medicare)
  • Employed or live with a family member who is employed
  • Permanent resident of Wake County

Mariam Clinic

  • Must not qualify for public or private insurance
  • Permanent resident of Wake County
  • Household income no more than 200% of Federal poverty level

  • Must be a non-pregnant adult
  • Must have no access to health insurance (Medicaid, Medicare, Private Insurance)
  • Permanent resident of Wake County
  • Household income no more than 250% of Federal poverty level
